Cohen and his colleagues have gained [practical knowledge of how to improve medication safety through more than 30 years of experience and hundreds of visits to health care organizations, often after a serious accident has occurred. This book attempts to summarize what has been learned from those experiences and to give providers a detailed blueprint for improving medication safety.
